Admission Form is available from at the Office of Dhaka Dental College, Mirpur-14 on cash payment of Tk. 500/. Admit Card will be given from office of the principal. Tk.1000/- have to pay at the time of application submission for Admission test fee.

Requirements:

At least one year should pass as after completion of internship.
Govt. candidates should complete 2 years of service.
Age of the applicants should not exceed 45 years.
Candidates employed at Govt/Semi Govt/Private/Autonomous organization should apply through authority and Permission of the authority must be submitted with the application.
Candidates holding FCPS (Oral and Maxillofacial Surgery) may apply directly to MS (OMS) Part-III. They have to face an interview board. They need not sit for written test, Candidates, provide incomplete application, false information (whenever detected) will be rejected.

Service (Revenue), Copy of first Joining Letter of Govt. Service (Revenue), Copy of first Appointment Letter of Govt. Service (Project), [if any], Copy of first Joining Letter of Govt. Service (Project), [if any], Copy of first Training Post of Govt. Service (If trained as a Govt. Officer), Copy of first joining letter of Govt. Service ( If trained as a Govt. Officer), Copy of last deputation order (If any), Copy of Post-graduate or Diploma (If any), 03 (three) copies of recent passport size photograph.
